L'endothéline est-elle impliquée dans l'hypertension ?

Les endothélines sont de puissants
vasoconstricteurs, possédant
également des propriétés mitogènes
et hypertrophiques. Beaucoup
d’études ont essayé de faire jouer
un rôle dans l’hypertension
artérielle à l’endothéline-1, mais
les réponses vasculaires à ce
peptide sont normales ou réduites
dans la plupart des modèles
d’hypertension expérimentale, et
dans l’hypertension essentielle
humaine. Ce n’est que récemment
que l’endothéline a pu être mise en
cause dans le modèle expérimental
d’hypertension induit chez le rat
par administration de sel et du
minéralocorticoïde
désoxycorticostérone (hypertension
DOCA-sel). En outre, les rats
spontanément hypertendus traités
par la DOCA et du sel
développent une hypertension
maligne accompagnée d’une
hypertrophie vasculaire sévère et
d’une surexpression de
l’endothéline-1 vasculaire ; ces rats
répondent alors aux antagonistes
de l’endothéline par une baisse de
leur pression artérielle. Dans
l’hypertension essentielle humaine,
il n’y a encore que peu d’éléments
en faveur de l’activation du
système de l’endothéline. En
conclusion, un rôle de
l’endothéline devient de plus en
plus apparent dans des formes
sévères d’hypertension
expérimentale, mais il faudra des
études supplémentaires pour
démontrer un rôle des endothélines
dans l’hypertension humaine. The endothelins, 21-amino acid peptides produced by the endothelium of blood vessels and many other tissues such as the kidney, brain, endocrine organs, etc, are potent vasoconstrictors, and are also endowed with mitogenic and cell hypertrophic properties. These peptides may be involved in the pathogenesis of hypertension through vascular, renal, endocrine and neural effects. Although many studies have been performed to test the hypothesis that these peptides have a pathophysiologic role in hypertension, it is only recently that evidence has been found of enhanced production of endothelin-1 in some experimental models of hypertension, particularly in blood vessels of rats rendered hypertensive by administration of salt and the mineralocorticoid deoxycorticosterone (DOCA-salt hypertension). Vascular responses to endothelin-1 have been shown to be normal or depressed in many models of experimental hypertension, and in essential hypertension in humans. In DOCA-salt hypertensive rats, which overexpress the endothelin-1 gene in the endothelium of large and small systemic arteries, chronic treatment with endothelin receptor antagonists blunted the elevation of blood pressure and development of vascular hypertrophy. Spontaneously hypertensive rats do not overexpress vascular endothelin, and do not exhibit a hypotensive response to chronic endothelin receptor antagonism. Malignant DOCA-salt-treated spontaneously hypertensive rats exhibit vascular overexpression of endothelin-1 and respond to endothelin antagonists with lowering of blood pressure. In human essential hypertension there is as yet little evidence of activation of the endothelin system. A role of endothelins in hypertension is thus becoming increasingly apparent in severe forms of experimental hypertension, but further studies are required to establish whether these peptides are involved in human hypertension.
